The Aging Well, Planning Well initiative is part of the Abbotsford Division of Family Practice Older Adults with Complex Conditions Shared Care project.
The project has multiple partners and supports doctors, other health professionals, and community leaders to better coordinate care for older adults (~60+) with complex health conditions. The project is focused on advance care planning and the prevention and management of 5 health conditions: heart disease, diabetes, chronic obstructive pulmonary disease (COPD), dementia, and depression.
Multiple partners were working towards hosting a large Aging Well, Planning Well education event for seniors but then the COVID-19 pandemic occurred. The project committee decided to instead provide a lot of the same educational information through a magazine, website, and videos.
